GRADUATION TRANSITION PLAN
2011 – 2012
The following three requirements must be met in
Grade 10:
1.
Transition
Plan
·
You will complete a draft of your Transition Plan this year in Planning 10. You must update this in
Grade 12, so it is important that you keep a copy saved on your personal
computer. You cannot hand this assignment in until Grade 12.
2.
Daily
Physical Activity
·
By successfully completing PE10 you will fulfill your Daily Physical Activity requirements
(150 minutes / week) for your Grade 10 year
3.
Healthy Living Plan
·
You will complete your Healthy Living Plan in PE10. This will be kept on file and used to meet your Grade 12 requirements.
The following two requirements must be met in
Grade 11:
1.
Daily Physical Activity
·
If you are not in a school physical activity class (PE
/ Dance / Weight Training / Keep Fit/Marching Band, Hockey Academy, Ace-It Fitness) you must print this Physical Activity Log and start logging
your hours NOW. Hand this log into the office (114) at the end of each
month.
2.
Explore Work Experience
· Visit the Career Programs office (room 116A) and explore your Work Experience options
The following five requirements must be met
in Grade 12:
1.
30 hours of Work Experience
·
You
must have completed 30+ hours of work or volunteer experience and documented it
with the Career Programs office (116a)
2.
Daily Physical Activity
·
If you are not in a school physical activity class (PE
/ Dance / Weight Training / Keep Fit/Marching Band, Hockey Academy, Ace-It Fitness) then print this Physical Activity Log and start logging your hours NOW. Hand this log into the office
(114) at the end of each month.
3.
A Healthy Living Plan
·
You need to complete the Healthy Living Plan
assignment by Friday, October 28th, 2011. Print out the assignment and the booklet in
which you must write your responses.

You need to complete the Graduation Transition Plan
assignment by Friday, January 13th, 2012. Use your computer to complete the
assignment and print it out. There is no response booklet for this assignment.

5.
Attend an Exit Interview
·
All Grade 12’s must complete an Exit Interview. The
first half of the Grade 12 group will interview in the library on Thursday,
February 9, 2012 and the second half will interview on Thursday, March 8th,
2012. You will dress smartly for an interview and bring your Graduation
Transition Plan. Be prepared to reflect on your life so far, and explain your
future plans to the interviewer. Further information will be provided closer to the time.
